Your initial appointment will consist of a consultation. During this appointment Dr. Comiskey will review your medical history, perform a comprehensive exam, discuss your treatment options, review surgical risks, and discuss your various anesthesia options, as well as answer all of your questions. You will also meet with our insurance specialist to review your insurance benefits. Most surgeries will require a second appointment although some procedures can be performed the same day as the consultation.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable (many xrays will be received via email).
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
- The above information can all be submitted online under the patient registration tab.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.
If your dentist or physician has taken x-rays, you may request that they forward them to our office.
The majority of xrays will be requested by our office and be received via email prior to your consultaiton appointment.
Additional xrays or a cone beam CT scan may be necessary and be taken in our office.
